Intra Cytoplasmic Sperm Injection (ICSI)
ICSI is Intra Cytoplasmic Sperm Injection. This refers to the laboratory procedure where a single sperm is picked up with a fine glass needle and is injected directly into each egg.
ICSI is Suitable for?:
Sperm Count is Very Low:
When sperm count is significantly low, making it difficult for natural fertilization to occur, ICSI provides a direct method of injecting a single sperm into the egg, bypassing the need for sperm to penetrate the egg on its own. This ensures that even with a low sperm count, fertilization can still take place successfully.
Sperm Cannot Move Properly or are Abnormal:
In cases where sperm motility is impaired or sperm exhibit abnormalities in shape or structure (morphology), conventional fertilization methods may be ineffective. ICSI allows for the selection of the healthiest sperm and facilitates their direct injection into the egg, overcoming obstacles that may hinder natural fertilization.
High Levels of Antibodies in the Semen:
Elevated levels of antibodies in the semen can interfere with sperm function and fertilization. With ICSI, sperm are isolated from the surrounding seminal fluid, reducing the impact of antibodies and enhancing the chances of successful fertilization by directly injecting a viable sperm into the egg.
Previous Fertilization Failure using Conventional IVF:
In cases where previous attempts at fertilization using conventional IVF have resulted in failure, ICSI offers an alternative approach with higher success rates. By directly injecting sperm into the egg, ICSI bypasses potential barriers to fertilization and increases the likelihood of achieving embryo development and implantation.
Sperm Retrieved Surgically from the Epididymis (MESA/PESA) or the Testes:
Surgical sperm retrieval procedures, such as Microsurgical Epididymal Sperm Aspiration (MESA) or Percutaneous Epididymal Sperm Aspiration (PESA), may be necessary for individuals with obstructive azoospermia or other conditions that prevent sperm from being ejaculated naturally. ICSI enables the use of surgically retrieved sperm by facilitating their direct injection into the egg, regardless of their origin.
ICSI Procedure
A semen sample with very low sperm count or the motility is very much on the lower side. In this situation, of course, we can harvest some sperm from the testes of the man. This sperm can be utilized to produce an embryo through a special procedure called ICSI or Intra-Cytoplasmic Sperm Injection.
In case of ICSI, We select the best sperm from the semen sample that is available, or the testicular sperm and we inject that individual sperm into individual egg. This gives rise to an embryo and that embryo is then re-positioned into the woman’s womb to create a pregnancy. Success rates are similar to a classic IVF procedure. But through the classic IVF procedure, we had much lesser fertilization rates or the embryo formation rates. This obstacle can be overcome with the help of ICSI procedure.
